Welcome to the support page. If you are seeking to process a defective item please download and fill out the following RMA form (52KB) Adobe reader required). Please read the directions thoroughly.

To receive an RMA number, please fill out this form completely and fax to (610) 695-0397 along with the copy of the invoice(s) for the items you wish to return. Be sure to list all serial numbers and include a detailed explanation for the reason of the return. Thank you for your cooperation.

RMA Guidelines

  • Please include a copy of all invoices
  • All shipping discrepancies must be reported to the customer service department upon receipt
  • Do not return manuals, Disks, Accessories or boxes. They will not be replaces. Return the defective item only.
  • Usage outside of the manufacturer specifications will void warranty.
  • Any item(s) not purchased from APEX will be returned
  • Any product received with physical damage or permanent markings will be refused.
